School Health Requirements (2020-2021 school year)

 

Preschool 3yr. old and Preschool 4yr. old

Iowa Immunization Requirements:

DPT (diphtheria/pertussis/tetanus): 4 doses

Polio: 3 doses

Hib (haemophilus influenzae type B): 3 doses with final dose given on or after 12 months of age

Pneumococcal: 4 doses

MMR (measles/mumps/rubella): 1 dose received on or after 12 months of age

Varicella (chickenpox): 1 dose received on or after 12 months of age

Kindergarten:

Iowa Immunization Requirements:

DPT (diphtheria/pertussis/tetanus: 5 doses with at least one dose on or after 4 years of age

Polio: 4 doses with at least one dose on or after 4 years of age

MMR (measles/mumps/rubella): 2 doses with the 1st dose given on or after 12 months of age and the 2nd dose given no less than 28 days after the 1st dose

Hepatitis B: 3 doses

Varicella (chickenpox): 2 doses received on or after 12 months of age, unless applicant has a reliable history of natural disease

Dental Screening Requirement:

All children enrolled in Kindergarten are required to have a dental screening. A screening for a Kindergarten student may be performed by a licensed dentist, dental hygienist, physician, nurse, advanced registered nurse practitioner, or physician assistant. A screening for Kindergarten is valid from 3 years of age to 4 months after the enrollment date.

Mandatory Lead Test:

All children enrolling in Kindergarten are required to have at least one lead test.            

Vision Screening Requirement:

All students enrolled in Kindergarten are required to provide evidence of a vision screening. Screenings performed no  earlier than one year prior to enrollment and no more than six months after enrollment will be accepted.

Health Physical:

Health physicals are not mandatory but are highly encouraged.

3rd Grade:

All students enrolled in 3rd grade are required to provide evidence of a vision screening. Screenings performed no earlier than one year prior to enrollment and no more than 6 months after enrollment will be accepted.

MS/HS: Athletic physical

All student athletes in grades 7-12 must have a current athletic physical and concussion form on file at school.

7th Grade:

All students entering 7th grade must provide proof of having received both a Tdap vaccine and a Meningococcal conjugate vaccine. These vaccines must be completed prior to the 1st day of classes for the 2020-2021 school year.

9th Grade:

All 9th grade students are required to submit a dental screening form. The screening for 9th grade may be performed by a licensed dentist or dental hygienist only. A screening for 9th grade is valid from one year prior to enrollment to four months after enrollment.

12th Grade:

All students entering 12th grade, born on or after September 15, 1999, must receive two doses of Meningococcal conjugate vaccine; or 1 dose if received when the student is 16 years or older. Proof of vaccination must be provided prior to the 1st day of classes for the 2020-2021 school year.
